To make an appointment, please contact our office by phone. Our office hours are 8:00 am - 6:00 pm Monday, Tuesday, Wednesday, and Friday. Walk-ins are welcom.
Financial
We accept most insurance plans and our a contracted provider for United Healthcare, Medica, Blue Cross, Health Partners and many others. Co-payments are due at the time of your visit. We also accept check, cash, or credit card.
Cancellation
If you are unable to keep an appointment, we ask that you provide us with 24 hours notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
